Ichigo has made it into the realm of his zanpakuto--which seems to be significantly wetter than it used to be, to find that Old Man Zangetsu is now in fact Young Man Tensa Zangetsu, and it looks like he's going to prove to be a formidable opponent to Ichigo now.
We find out, also, the name of Isshin's zanpakuto--Engetsu, 'Crescent Moon'.
However, though Ichigo has made it to Tensa's realm, it doesn't mean things are gonna be easy and they'll be working together from here on out--in fact, Tensa doesn't seem to want to help Ichigo learn the move at all.
According to Tensa...
"Why should I care what happens to what you want to protect?
...Don't misunderstand, Ichigo...
What you want to protect and what I want to protect,
Are two completely different things..!"
What could this mean? Regardless, though, Ichigo's going to have to fight his way through this one.
Meanwhile in Karakura, it looks as though there are a handful of people, aside from Tatsuki and Keigo, who are in fact awake and walking around.
For their sakes, though, let's hope they don't all run into Aizen and Gin like one unfortunate gentleman has, because he's met his end now, thanks to the fact that

but spiritual bodies themselves cannot bear my power."
Seems Aizen killed an unfortunate bystander simply by walking close to him, only because the man's human body couldn't handle Aizen's spiritual pressure.

Tatsuki and Keigo probably should be, because Gin and Aizen have caught them on their radar now....
